To use, simply fill the douche-bulb up with warm water (not hot, you'll scorch your backside!), secure the attachment to the douche - should slide into the hole at the top of the bulb. Then insert the attachment up the bum and then start squeezing the water up into your bowels to begin stage 1 of having a squeaky clean backside.
Should you mix the water with shower gel / soap? Not straight away. Sometimes soap-based products can cause irritation, so wait until you're more experienced at douching before experimenting with soap and if you feel any irritation you know to stick to warm water only.
